This data includes the hospitalization rates for Angina, Asthma, COPD, Diabetes, Heart Failure and Hypertension for each municipality in the MAPC region
between 2003 and 2008. All values are Age Adjusted Rates of Hospitalizations per 100,000.

This data shows the age adjusted lead poisoning occurrances rate of hospitalization per 100,000 people. The data is divided into 3 groups: Elevated Blood Levels (>=15 µg/dL), Immediate Risk Blood Levels (20-24 µg/dL)
and Lead Poisoning Blood Levels (>=25 µg/dL) for each municipality in the MAPC region between 1998 - 2008.

This data includes the number of low birthweight babies (less than about 5.5 pounds) of singleton (not twins, triplets, etc) births for each municipality in the MAPC region
between 2003 and 2007. It also includes the percent of all singleton births that were low birth weight births during that time. Low birth weight has been found to increase
the risk of infant as well as adult health problems and to impede educational and developmental progress.

This data layer shows the number of residents living with HIV/AIDS in 2006 by municipality.
This layer also shows the crude rate of HIV/AIDS prevalence per 100,000 people, along with lower and upper estimates of this crude rate.
These data provide crucial information for monitoring HIV/AIDS in the Commonwealth. |

This point datalayer contains the location of community health centers (CHCs) in Massachusetts. The layer was produced by the Massachusetts Department of Public Health (MA DPH) Bureau of Environmental Health (BEH) GIS program. The source material was provided by Tina Ford Wright, Publications and Marketing Assistant, Massachusetts League of Community Health Centers, a.k.a. "the League," (http://www.massleague.org). The League defines a community health center as a non-profit community-based organization that offers comprehensive primary and preventive health care, including medical, social and/or mental health services, to anyone in need regardless of their medical status, ability to pay, culture or ethnicity (data description from http://www.mass.gov/mgis/chcs.htm).
